Estate Plan Questionnaire

This is the questionnaire for your Will & Estate Plan. It takes about 15 minutes for an individual plan and about 20 for a couples plan. You can only submit it once, so have your details ready. If you need to change something later, email support@namechange.com within 60 days.

NameChange.com is not a law firm and does not give legal advice. This questionnaire produces self-help documents prepared from your answers. It is not a substitute for the advice of an attorney. If your situation is complex (a business, a blended family, property in more than one state, a very large estate), consider talking to an estate attorney first.
Before you start
About you

Use your full legal name exactly as it will appear on the day you sign.

Marital status
Your will: who inherits

Your "residuary estate" is everything you own at death that is not left to a specific person below. Most people leave it to their spouse first, then children.

Your will: who is in charge
Your financial power of attorney

The person you name can handle money and property for you if you cannot. Choose someone you trust completely.

When should it take effect?
Your healthcare directive
If you are terminally ill or permanently unconscious with no reasonable hope of recovery, what do you want?
Organ and tissue donation
Couples plan: your partner

Your partner's documents mirror yours: they name you first, then your children or the people you choose. Fill in only what differs.

When should your partner's power of attorney take effect?
Partner's wishes if terminally ill or permanently unconscious
