This ensures all your records match and helps prevent mismatches that could delay other updates.

✅ Step-by-Step: Change Your Name with the SSA

1. Complete Form SS-5

The form used to change your name with the SSA is called Form SS-5, also known as the “Application for a Social Security Card.”

2. Gather Required Documents

You’ll need to provide original or certified copies of the following:

  • Your legal name change document (marriage certificate, divorce decree, or court order)

     

  • Proof of identity: A current driver’s license, state ID, or U.S. passport

     

  • Proof of citizenship or lawful status, if your citizenship isn’t already established with SSA

     

Note: You do not need to provide proof of your old name—the SSA will use your current records for reference.

3. Submit Your Application

You can submit your application in person or by mail to your local Social Security office.

There is no fee to change your name.

4. Receive Your Updated Card

Once your name change is processed, you’ll receive your new Social Security card by mail within 7–14 business days. Your Social Security number will not change, only your name.

5. Wait 48–72 Hours Before Updating Other Records

The SSA system must be updated before other agencies—like the Michigan SOS—can verify your new name. Wait at least 48–72 hours before trying to update your driver’s license or other federal/state records.
