Step-by-Step Instructions
- Visit Your Local BMV Office You must appear in person to update your name on the vehicle title. Use the BMV Office Locator to find the closest branch.
- Bring These Documents:

Your current Ohio title

Certified copy of your legal name change order or marriage/divorce certificate

Valid Ohio driver’s license or state ID with your new name

Application for Certificate of Title to a Motor Vehicle (Form BMV 3774)
- Pay the Fee As of 2024, the fee for a duplicate title is $15.00. Some counties also charge a small processing fee.
- Update Registration (Optional but Recommended) You can update your registration at the same time. If your name change affects your plate or insurance, this is a good time to update everything at once.
