Step-by-Step Instructions

1. Visit Your Local BMV Office You must appear in person to update your name on the vehicle title. Use the BMV Office Locator to find the closest branch.
2. Bring These Documents:

• Your current Ohio title

   

• Certified copy of your legal name change order or marriage/divorce certificate

   

• Valid Ohio driver’s license or state ID with your new name

   

• Application for Certificate of Title to a Motor Vehicle (Form BMV 3774)

   

3. Pay the Fee As of 2024, the fee for a duplicate title is $15.00. Some counties also charge a small processing fee.
